Hi
I have the same issue, i tried dropping the DB and replaced all the changed files, restarted the install and got the same problem. I am using MYSQL 5.0 but from the forum I can tell that isn't the problem.
Lines 497-499 read:
// popuplate blocks table
$result = $inst_dbconn->Execute("INSERT INTO ".$prefix."_blocks VALUES (1,'menu','$mainblock_block_title',
Appreciate any ideas on how to figure this out.

Hi Larsneo
Yes I tried all the suggestions (using .htaccess, php.ini and the download, my host is 1and1.co.uk and I contacted them about the register globals and they told me to put register_globals = off in a php.ini file in the root but that didn't work either.
So I just trashed everything downloaded the zip again and without changing anything it worked. No idea what I did differently but now it's installed.
